Service Agreement

 

Thank you for choosing Esteem Body Contour By Kei, LLC for your body contour and body sculpting services. We value our clients and want to ensure that you receive quality care and reach your body goals. We ask that you read and acknowledge the following service contract, so that we ensure you have the best possible experience.

Appointments

ARRIVAL

Please arrive to all appointments at your scheduled time. We ask that you do not come into the studio early as there is not a waiting area, and we want to ensure that privacy is maintained for all of our clients. During your initial appointment you will be required to fill out basic intake forms similar to the forms you will submit prior to scheduling an appointment.

LATE ARRIVALS

There is a 10 minute grace period for late arrivals to your appointment. After 10 minutes lateness may seriously impact the quality of your treatment and we will not be able to service you. Any lateness past 10 minutes will result in cancellation of the appointment and a $25 cancellation fee, in addition to the forfeit of deposit. Any fees must be paid prior to rescheduling.

RESCHEDULING

If you are unable to make your scheduled appointment, we ask that you reschedule within at least 24 hours of the appointment time. Failure to provide notice and reschedule will result in a cancellation fee of $25 in addition to the forfeit of deposit. Any fees must be paid prior to rescheduling.

CANCELLATIONS

Our cancellation policy requires 24-hour notice for all appointments. Notice is considered by phone call, text, or email. In the event that proper notice as stated above is not given a fee will be implemented of $25 in addition, the forfeiture of deposit, booking fees, and full payments received by the client. All monies received by the client are non-refundable and non-transferrable. It is the owner’s discretion to waive this policy or give warning. Any fees must be paid prior to rescheduling.

NO SHOWS

We are dedicated to you and ask that you respect the time that has been devoted to your care. NO SHOWs are disruptive to the schedule and take away from others who could have taken that slot. NO CALL NO SHOW will incur a $25 fee in addition to the forfeiture of any monies received including deposit and full payment of service if paid in full. A warning will be given for the first offense and the client will allowed to schedule again. Repeat NO SHOW will incur the above and be banned from scheduling any further service with the company indefinitely unless stated otherwise by the company owner. Any fees must be paid prior to rescheduling.

AFTER HOURS APPOINTMENTS

After hours appointments are scheduled at the discretion of the owner. There is a $50 additional fee required.

DEPOSITS

There is a non-refundable deposit required to secure an appointment. Your appointment is not confirmed until the deposit has been paid. Please pay your deposit using PayPal or Square. The remaining balance may be paid via PayPal, square, invoice, apple pay, or cash in studio. Specials, packages and promotions must be paid for in full at time of initial booking. When using financing, subject to the terms and conditions of the financing agreement. Chargebacks will incur a $30 fee and may result in the client not being allowed to use that form of payment in the future.

APPOINTMENT REMINDER

An appointment reminder will be sent out within 24 hours of your scheduled appointment via email and text. Initial appointments will receive a personal text to the number on file from the owner or its representative with instructions to follow for initial visit.

Sales and Fees Policies

FORMS OF PAYMENT ACCEPTED

We accept Mastercard, VISA, Discover, American Express, Cash, PayPal, Square and Apple pay. We do not accept insurance or HSA. If paying with Cash, we only accept exact change. We do not accept checks or money orders.

WHEN IS PAYMENT EXPECTED

Payment is expected at the time of the initial service. Deposits are required to secure appointment time. Payment will be required up front and should be remitted at the time of entry to the studio. Packages and specials must be paid in full prior to end of series/service.

REFUNDS

Services and fees are nonrefundable and non-transferrable. In the event that a client has paid in full for service and chooses to cancel or is cancelled by the company for any reason prior to receiving services, fees minus the deposit will be refunded. All deposits are nonrefundable and nontransferable.

DEPOSITS

A deposit is required to secure an appointment. Deposits are nonrefundable. Deposits are non-transferrable in the event of a NO SHOW, or late cancellation. In the event of a NO SHOW or late cancellation, deposit is forfeited, and a new deposit will be required to schedule a new appointment in addition to any other fees accrued.

GIFT CERTIFICATES

Gift certificates are available for purchase. There are no refunds or cash back on gift certificates. Gift certificates will expire within 12 months from the date of purchase.

PRODUCT RETURNS

There will be no refunds for products purchased. An in store credit will be issued for any products returned unused and in original packaging within 7 days of purchase.

Operations/ General Policies

CELL PHONES

At Esteem Body Contour by Kei, LLC we are dedicated to providing a calm and relaxing environment. We ask that cell phones be silenced while in the studio. Clients are permitted to take personal photos of themselves while inside the studio. No photos of the studio, equipment, products, tools, machines, and/or workers are permitted by any means for any reason. We ask that you do not take calls during treatments, and if absolutely necessary it be cleared with the tech/nurse prior to the call. Rescheduling of appointment may be necessary to ensure that treatment is not disrupted. Any breach in the above policy may result in the dismissal of the client, cancellation of services, and/or legal action to be taken.

CHILDREN/EXTRA GUESTS

Children and extra guests are not permitted. Only clients scheduled for service will be permitted in the studio.
